Support coordination · Sydney North Shore

Support coordination on the North Shore.

A coordinator who already knows the local providers, the waitlists, and which ones actually call you back. Based on the Pacific Highway at St Leonards, working from Crows Nest to Hornsby.

Answered Mon–Fri 9:00am–5:00pm.

Support coordination

What a coordinator actually does

Support coordination is the difference between having funding and having supports that started. Most of the work is unglamorous: phone calls, follow-ups, and knowing which provider on the North Shore has capacity this month rather than a waitlist.

What’s included

  • Reading your plan with you and explaining what each budget can and cannot pay for
  • Finding and vetting providers who have capacity now, not in six months
  • Setting up service agreements and getting the first bookings in the diary
  • Chasing providers who go quiet, so you are not the one making those calls
  • Getting your evidence together well before your plan reassessment
  • Reporting to the NDIA on your behalf when that is part of the funding

How it works

Getting started

01

A call about your plan

Thirty minutes, no charge. We go through what is funded and what you actually want to happen.

02

A shortlist, usually within a week

Real options with real availability, and an honest word on the ones we would not use ourselves.

03

Agreements and first bookings

We handle the paperwork and the back-and-forth, and tell you when something is booked.

04

Check-ins that keep going

Supports drift. We stay on it, and we act before the plan reassessment, not after.

Our patch

We know these streets, because we work on them

Between the hospital precinct, the station and the bush tracks at Lane Cove, the North Shore isn’t one place. Support that works in Crows Nest doesn’t automatically work in Wahroonga.

Meet & greet

Meet your support worker before you decide

Nobody should have to pick a provider off a website. Come and meet the person who’d be turning up. Have a coffee. Ask the awkward questions. If it isn’t a fit we’ll find someone else, and it costs you nothing either way.

Who this is for

Ring us if any of these sound like you

  • You have Support Coordination funded and have not used it yet
  • Your current coordinator has stopped answering
  • This is your first plan and you do not know where to start
  • Your plan is up for reassessment in the next few months

Who you’ll be dealing with

Small team. Same faces.

You won’t get a different worker every fortnight, and you won’t be handed to whoever is free. The person you meet is the person who turns up.

Common questions

Support coordination

How do I know if support coordination is funded in my plan?

It sits under Capacity Building, usually as “Support Coordination” or “Specialist Support Coordination”. Send us the plan or read it out over the phone and we will tell you in a couple of minutes, at no cost.

Can I change coordinators in the middle of a plan?

Yes, and you do not need the NDIA’s permission. You end the service agreement with the current provider and start one with us. We can do the handover so you are not stuck in the middle of it.

What is the difference between support coordination and plan management?

A plan manager pays your invoices. A support coordinator finds and organises the supports in the first place. They are separately funded and you can have both.

Do you charge for travel across the North Shore?

Travel is chargeable under the NDIS price guide, and we will tell you upfront what that looks like for your suburb before you sign anything.

Which level of support coordination do I have?

There are three. Support Connection is short-term help to get started. Support Coordination is the common one, ongoing help to find providers, set up services and keep them running. Specialist Support Coordination is for high-risk or crisis situations and is delivered by someone with clinical qualifications. Your plan names which one you have. Read it out to us and we will tell you.

Does support coordination eat into my other funding?

No. It is funded separately under Capacity Building and sits on top of your Core budget. Using your coordination hours does not reduce what you have for support workers, therapy or anything else.

How often will I actually hear from you?

We agree a rhythm at the start, for most people that is a check-in every two to four weeks, plus whatever the current problem needs. If you have not heard from us when you expected to, that is our fault, and you should ring and say so.

Can I have a support coordinator and a recovery coach?

Usually not both. The NDIA generally funds one or the other, because a psychosocial recovery coach does the coordination work as part of the role. If your main barrier is mental health, the recovery coach is normally the better fit, see our recovery and reintegration page.

You deliver supports as well as coordinating them. Isn’t that a conflict?

It can be, so we will say it plainly: we will always give you options that are not us, in writing, and we will never make using our own services a condition of coordinating your plan. If you would rather have a coordinator with no service arm at all, say so and we will point you to independent ones on the North Shore.

Do you take self-managed and plan-managed participants?

Yes. Self-managed, plan-managed and NDIA-managed. If you’re not sure which you are, tell us and we can work it out from your plan.

How soon can support start?

It depends on the support and the hours you need. Call and we’ll give you a straight answer on availability rather than putting you on a list.

Can I meet the support worker before we start?

Yes, and we’d encourage it. Meet whoever would be supporting you before anything is signed. If the fit isn’t right, we find someone else.

What if I’m not on the NDIS yet?

That’s exactly what our NDIS Navigators do. Eligibility, evidence, the Access Request, all before you have any funding.

Are you a registered NDIS provider?

Yes, Thrive Ability Services is a registered NDIS provider.

I’m a support coordinator. How do I refer someone?

Use the referral form or call 0421 959 006. We’ll come back to you on capacity and likely start dates.
